Providing healthcare options for your employees doesn’t have to be complicated or expensive. Our Employer Healthcare Plans offer flexible, affordable options to help businesses support their employees’ well-being.
Additional visits can be purchased at any time. 
Option 1: Standard Plan

($550 per Employee Per Year)

Each enrolled employee receives 1 physical, including lab panel and 4 office or telehealth visits to Summit View Primary Care.
Visits can be used for routine checkups, acute illnesses, or chronic condition management.
Employers can enroll as many employees as they wish.
Additional visits can be purchased at any time. 
Option 2: Flexible Plan

($550 for 5 Visits Per Year)

Employers receive 5 visit credits.
These visit credits can be distributed among employees as needed.
Ideal for businesses with employees who may need more flexible healthcare access.

Option 3: Flexible Plan

($1050 for 10 Visits Per Year)

Employers receive 10 visit credits
These visit credits can be distributed among employees as needed.
Ideal for businesses with employees who may need more flexible healthcare access.

Option 4: Flexible Plan

($2000 for 20 Visits Per Year)

Employers receive 20 visit credits.
These visit credits can be distributed among employees as needed.

Employee Out-of-Pocket Costs for Medications & Lab Work

While the employer plan covers primary care visits, employees will be responsible for the cost of any medications or lab work that may be required. We provide transparent and affordable pricing:

Medications:

Many common prescriptions are available at discounted rates, with prices as low as $10 for a 90-day supply for certain medications.

Lab Work

Routine lab tests such as blood work, cholesterol panels, and diabetes screenings start at $6 per test, with comprehensive screenings starting at $50.

Custom Pricing

We always discuss the cost of any medications or lab work before proceeding, ensuring no surprise bills.
